Application Privacy Policy (“Google Play”)

This Mobile App Privacy Policy, along with the AllWays Member Terms of Use will tell you what information we collect through AllWays Member and how it is used.

In hosting the app for download, Google (through the Google Play) may have access to a limited amount of the user’s information, provided directly by the user (including but not limited to name, email, IP address, and the app downloaded).  However, Google does not collect such information for or on behalf of ALLWAYS, and it will not be subject to our Member Privacy Practices or this app’s Application Privacy Policy.  By agreeing to these terms, you acknowledge that information provided to Google by the user (and not by the app or by ALLWAYS) does not constitute protected health information under the Health Insurance Portability & Accountability Act and related regulations (collectively referred to as “HIPAA”) and as defined herein.

In addition, AllWays Member may use analytics and other services and tools services developed internally or provided by third parties to collect certain information relating to your use of AllWays Member. We and our service providers may also from time to time use the application and other technologies contained within the application to receive and store certain types of information whenever you interact with the application through your mobile device.  By agreeing to these terms, you acknowledge that information collected by these tools does NOT constitute protected health information under the Health Insurance Portability & Accountability Act and related regulations (collectively referred to as “HIPAA”) and as defined herein.

Except as set for in the previous two (2) paragraphs, information that you may provide to us in connection with the use of this application is considered “Protected Health Information” as defined under HIPAA and is subject to our Member Privacy Practices.

By using the AllWays Member app you agree to acceptance of this Mobile App Privacy Policy and our Member Privacy Practices.

Use of Third-Party Services; Information Collected Though Interaction with the Application 

Information We Collect

You will be asked to provide personal information (including certain demographic and contact information, including email addresses) during the AllWays Member registration process, during the AllWays Member login process, or when providing feedback to us about your experiences using AllWays Member. The information required is noted on each specific form or registration page. Additional questions might be included to help us improve the services we offer. Any information collected through the AllWays Member is not stored on the website but will go to back-office systems in the same manner that information from paper forms, submissions, and phone calls to customer care are currently handled. At no time will AllWays sell, rent, loan, trade, or lease any information collected on AllWays Member. However, please note that we may need to use this information within the AllWays family of affiliate companies or with a third-party vendor to perform  administrative, technical, or other functions that help us provide service to you. We seek to hold third-party vendors to the same privacy practices and protections as AllWays.

Cookies, Tracking and Internet-based Advertising

This application works with third-party products or services (“Service Providers”). These include Google and other Service Providers who help us track and analyze visitor activity on the application, measure the effectiveness of our advertising efforts, and support the optimization of our digital marketing campaigns.

The application uses “cookies,” tracking pixels and related technologies. These can be set by ALLWAYS or by our Service Providers. 

Cookies are small data files that online services can store on, and retrieve from, a user’s computer or mobile device through the user’s web browser. The information is stored and retrievable either for the duration of an application visit (known as “session cookies”), or until some later point in time set by the application setting the cookie (known as “persistent cookies”).

Examples of information we and our Service Providers collect include how often someone visits this application and what they do while on the application. We and our Service Providers use this information in the aggregate to understand how our visitors as a group use different resources and to help us improve our application. We and our Service Providers also collect information about the activity of individual application users. This information may be used to direct related ALLWAYS advertisements and/or informational content to application users when they visit other applications.

Most internet browsers accept cookies by default, but you can modify your browser preferences to block or restrict cookies if you do not want your web browsing activity tracked. We are not responsible for the effectiveness or compliance of third party opt-out mechanisms or programs. Please note that if you delete your cookies or upgrade your browser after having opted out, you will need to opt out again. Further, if you use multiple browsers or devices you will need to opt out on each browser or device.  You can access the information on this application without enabling cookies in your browser, but disabling cookies may result in a diminished ability to take advantage of the services and related informational content on the application.

We use web and application analytics and tag management services and tools provided by Google and other third parties to collect certain information relating to your use of this application. Our Service Providers may acquire additional information about your activity on our application, including pages you visit, access times, visit duration, how you arrived at our application and your IP address. An IP address is a number that identifies a device connected to the Internet. For most devices, the IP address changes on at least a weekly basis.  Our Service Providers may also acquire device identifiers and specific information about the browser you use. In some cases, this information may be unique to you. 

Biometric Login

Enabling biometric login allows you to use your fingerprint, facial, or other physical character recognition instead of your username and password to access and use the AllWays Member if your device is equipped with biometric login functionality. In order to use biometric login to access and use the AllWays Member, you need to first enable Biometric login functionality on the device. BY SPECIFICALLY ENABLING BIOMETRIC LOGIN TO ALLWAYS MEMBER, YOU ARE ALLOWING AND AUTHORIZING ANYONE WHOSE BIOMETRIC IDENTIFICATION IS STORED ON YOUR DEVICE TO ACCESS ALLWAYS MEMBER, VIEW YOUR ALLWAYS MEMBER INFORMATION, AND MAKE ALLWAYS TRANSACTIONS. ACCORDINGLY, YOU WILL BE RESPONSIBLE FOR SUCH TRANSACTIONS EVEN IF YOU DID NOT MAKE THEM, OR EVEN KNOW ABOUT THEM. THEREFORE, IT IS STRONGLY RECOMMENDED THAT YOU NOT ENABLE BIOMETRIC LOGIN FOR ALLWAYS MEMBER IF ANYONE OTHER THAN YOU HAS A BIOMETRIC LOGIN STORED ON YOUR DEVICE. BY AGREEING TO THESE TERMS AND ENABLING BIOMETRIC LOGIN FOR ALLWAYS MEMBER, YOU ARE AUTHORIZING ANYONE WITH BIOMETRIC DATA STORED ON YOUR DEVICE TO ACCESS THE ALLWAYS MEMBER APPLICATION, VIEW INFORMATION ACCESSIBLE THROUGH THE ALLWAYS MEMBER APPLICATION AND MAKE ACCOUNT TRANSACTIONS ON YOUR BEHALF, FOR WHICH YOU WILL BE LEGALLY RESPONSIBLE, AND REPRESENTING TO US THAT ANY SUCH PERSON HAS PROPER AUTHORITY TO MAKE SUCH TRANSACTIONS ON YOUR BEHALF. Note that we do not have access to your biometric data. For more information on how the device manufacturer or operating system provider uses and stores biometric data, refer to those companies’ privacy policies and/or security guidelines. Biometric technology is provided by such manufacturers and providers, and accordingly we are not responsible for any malfunctions or errors.

How We Use This Information

AllWays uses the information you provide to operate AllWays Member and provide the AllWays Member service to you. In addition, AllWays may also use certain personal information, including your email address, to provide communications, information and materials to you related to AllWays Member and/or other programs and services available through AllWays and certain AllWays affiliates, and certain information and surveys, including member satisfaction surveys relating to your use of services provided by AllWays and certain AllWays affiliates. AllWays may also share such information with third party service providers whose services and/or materials are accessible through AllWays Member to enable such third parties to provide those services and materials to you.

Data Security

AllWays seeks to prevent unauthorized access to your personal information by use of security technologies to protect the integrity and privacy of any personal information you provide.

Although we endeavor to protect your personal information, AllWays cannot and does not warrant the security of any information you transmit to us through online applications, and you do so at your own risk. We use industry standard physical, technical and administrative security measures and safeguards to protect the confidentiality and security of your personal information. However, since the Internet is not a 100% secure environment, we cannot guarantee, ensure, or warrant the security of any information you transmit to us. There is no guarantee that information may not be accessed, disclosed, altered, or destroyed by breach of any of our physical, technical, or managerial safeguards. It is your responsibility to protect the security of your login information and the devices used to access AllWays Member.

Depending on the nature of the inquiry, your communication could be discarded or archived. If you wish, you can contact us instead by telephone at the numbers provided throughout our site.

Login/Passwords

Some sections of the AllWays Member website require login identifiers and/or passwords. You are responsible for taking reasonable steps to ensure that no unauthorized person has access to your password or account login information. It is your sole responsibility to control the use of login information and passwords, to authorize, monitor and control access to and use of your account and password and to inform AllWays Member promptly of any need to deactivate a password.

No information from children under age 13

If you are under the age of 13, please do not attempt to register with us at allwayshalthpartners.org or provide any personal information about yourself to us. If we learn that we have collected personal information from a child under the age of 13, we will promptly delete that information. If you believe we have collected personal information from a child under the age of 13, please contact us.

Legal proceedings

We will share personal information with third-party companies, organizations, or individuals outside of AllWays and its affiliates if we have a good-faith belief that access, use, preservation, or disclosure of the information is reasonably necessary to:

Transfer in the event of sale or change of control

If the ownership of all or substantially all of our business changes or we otherwise transfer assets relating to our business or the website to a third party, such as by merger, acquisition, bankruptcy proceeding, or otherwise, we may transfer or sell your personal information to the new owner. In such a case, unless permitted otherwise by applicable law, your information would remain subject to the promises made in the applicable Website Privacy Policy unless you agree differently.

If you use our website and provide us with information, you understand and agree that your information may be transferred to and stored on servers located outside your resident jurisdiction and, to the extent you are a resident of a country other than the United States, that you consent to the transfer of such data to the United States for processing by us in accordance with this Privacy Policy.

Changes to This Policy

AllWays reserves the right to change or update this Mobile App Privacy Policy from time to time without notice, so please review it periodically to keep informed of any changes.
